Start your application the day you receive&nbsp;your notice. If your SIN begins with a 9, you need to supply&nbsp;<\/span>proof of your immigration status and work permit.<\/div>\n<\/li>\n<li><span class=\"pg-1ff1\">your mother\u2019s maiden name.<\/span><\/li>\n<li><span class=\"pg-1ff1\">your mailing and residential addresses, <span class=\"pg-1ff2\">including the postal codes<\/span>.<\/span><\/li>\n<li><span class=\"pg-1ff1\">your complete banking information to sign up for direct deposit, including the financial&nbsp;institution name, bank branch number, and account number<\/span><\/li>\n<li><span class=\"pg-1ff1\">names, addresses, dates of employment, and reason for separation for all your employers&nbsp;over the last 52 weeks<\/span><\/li>\n<li class=\"t pg-1m0 pg-1x2 pg-1h3 pg-1y9 pg-1ff1 pg-1fs0 pg-1fc0 pg-1sc0 pg-1ls0 pg-1ws0\">\n<div class=\"t pg-1m0 pg-1x3 pg-1h3 pg-1y14 pg-1ff3 pg-1fs0 pg-1fc1 pg-1sc0 pg-1ls0 pg-1ws0\"><span class=\"pg-1ff1\">your detailed version of the facts (if you quit or have been dismissed from any job in the <\/span>last 52 weeks)<\/div>\n<\/li>\n<li class=\"t pg-1m0 pg-1x2 pg-1h3 pg-1y9 pg-1ff1 pg-1fs0 pg-1fc0 pg-1sc0 pg-1ls0 pg-1ws0\">\n<div class=\"t pg-1m0 pg-1x3 pg-1h3 pg-1y16 pg-1ff3 pg-1fs0 pg-1fc1 pg-1sc0 pg-1ls0 pg-1ws0\"><span class=\"pg-1ff1\">the dates, Sunday to Saturday, and earnings for each of your highest paid weeks of <\/span>insurable earnings in the last 52 weeks or since the start of your last EI claim, whichever is the shorter period. This information will be used, along with your Record(s) of Employment, to calculate your benefit rate.<\/div>\n<\/li>\n<\/ul>\n<p>If you are reactivating an existing claim:<\/p>\n<ul>\n<li class=\"t pg-1m0 pg-1x2 pg-1h3 pg-1y9 pg-1ff1 pg-1fs0 pg-1fc0 pg-1sc0 pg-1ls0 pg-1ws0\">\n<div class=\"t pg-1m0 pg-1x3 pg-1h3 pg-1y1c pg-1ff3 pg-1fs0 pg-1fc1 pg-1sc0 pg-1ls0 pg-1ws0\"><span class=\"pg-1ff1\">the salary amount you received, before deductions, for the last week you worked (from <\/span>Sunday to your last day of work), including insurable tips and commissions<\/div>\n<\/li>\n<li class=\"t pg-1m0 pg-1x2 pg-1h3 pg-1y9 pg-1ff1 pg-1fs0 pg-1fc0 pg-1sc0 pg-1ls0 pg-1ws0\">\n<div class=\"t pg-1m0 pg-1x3 pg-1h3 pg-1y1e pg-1ff3 pg-1fs0 pg-1fc1 pg-1sc0 pg-1ls0 pg-1ws0\"><span class=\"pg-1ff1\">any other amount you received or will receive (such as vacation pay, severance pay, <\/span>pension payments, pay in lieu of notice, and other money)<\/div>\n<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>You may have gotten a notice that you are now on Off Duty Status (ODS), or lay-off, here\u2019s&nbsp;what you need to know.
